Here are the simple steps to confirm your massage coverage details, and how to use your insurance in our office.
NOTE: Every insurance holder is required to pay cash for the first session. When coverage details are confirmed, payment balances will be forwarded to future copays/coinsurance fees.
Option 1 -- DIY
1. Print out the pdf or doc, "insurance".
2. CALL the member phone number on the back of your member card. Select 'representative' (talk to a human).
3. Ask for massage coverage information. With the rep's help, fill in the blanks on the insurance form.
4. Email or text:
a- a pic of the filled out form
b- pics of your insurance card, both sides
Option 2-- In Office
1. Make an appointment, and visit us!
2. Bring your insurance card to your appointment.
3. During your appointment, the LMP will call and confirm your coverage information.
